অামরা রোগীর General Examination করার সময় যতগুলো Sign দেখি তাদের মধ্যে Clubbing একটি গুরুত্বপূর্ণ sign. এখন কথা হলো, Clubbing অাসলে কী?
যদি Clubbing কে define করতে হয় তখন Definition টা দাঁড়ায় এরকমঃ Clubbing is a charecteristic bulging of distal fingers with obliteration of the angle between nail bed and nail fold.
Clubbing এর Exact mechanism এখন পর্যন্ত জানা যায়নি।
তবে কিছু Hypotheses প্রচলিত অাছে। এগুলোর মধ্যে Platelet Derived Growth Factor (PDGF) hypothesis বহুল প্রচলিত যদিও এটি Unilateral clubbing কে এক্সপ্লেইন করতে পারে না।
যাই হোক, অাজকে অামরা এই hypothesis টি নিয়েই কথা বলবো। এই hypothesis অনুসারে, bone marrow থেকে যে megakaryocyte অাসে সেগুলো mature platelet এ convert হয় Pulmonary circulation এ গিয়ে।
যদি কোনো কারণে megakaryocyte গুলো Mature platelet এ কনভার্ট হতে না পারে,তখন সেগুলো unfractionated অবস্থায় distal finger এর capillary তে গিয়ে capillary block করে দেয়।
তখন সেই unfractionated megakaryocyte থেকে অাসে Platelet Derived Growth Factor (PDGF) অার এই PDGF অাল্টিমেটলি Smooth Muscle cell এবং Fibroblast এর Proliferation করে।
ফলে একসময় distal finger এর bulging হয়, অন্যদিকে nail bed অার nail fold এর মাঝখানে যে 165 ডিগ্রী angle সেটাও থাকে না। একেই অামরা Clubbing হিসেবে দেখি। এছাড়াও, bowel disease এর ক্ষেত্রে Polycythaemia এবং arteriovenous malformation in lung Clubbing এর জন্য অনেকটা কন্ট্রিবিউট করে।
অাবার, Lung cancer এবং Hypertrophic Osteoarthropathy এর ক্ষেত্রে Vascular Endothelial Growth Factor (VEGF) এর অনেকটা কন্ট্রিবিউশন অাছে।

এখন অাসি Clubbing এর stage এ। অামরা সাধারণত Clubbing এর স্টেজকে ৫ ভাগে ভাগ করতে পারি।
#Stage_1: Softening of the nail bed causing the spongy feeling when the nail bed bed is pressed. #Stage_2: Loss of the normal angle 165 degree between nail bed and nail fold.
#Stage_3: Convex nail growth.
#Stage_4: Thickening of the distal finger.
#Stage_5: Shine and Striation in the nail and skin. এবার Clubbing এর cause এ।

Clubbing অনেকগুলো cause এর মধ্যে নিচের cause গুলো না বললেই নয়-
a)Neoplastic causes: 1. Bronchogenic Carcinoma 2) Lymphoma 3) Pleural Tumour
b)Pulmonary Causes: 1. Cystic Fibrosis 2.Asbestosis 3.Pulmonary Fibrosis 4. Amyloidosis
c)Cardiac Causes: 1. Cyanotic Heart Disease 2. Endocarditis
d)Gastrointestinal Causes: 1.Coeliac Disease 2. Chronic Liver Disease 3. Inflammatory Bowel Disease
e) Infectious Causes: 1. Tuberculosis 2. Subacute Bacterial Endocarditis 3.HIV f) Endocrine Causes: 1.Thyroid Disease
